I give permission to Swing High Golf to take photographs of me (my child) during events or activities. These images maybe used for promotional purpose including on websites, social media, and printed materials, and presentations.
* For Parents/Guardians of Student
I understand that I must stay at the facility while my child is practicing or taking part in any activity, even if volunteers are present. I agree not to leave the premises during this time.
